This camp will include one movie night and one pool night.

We need this form as soon as possible so we can get your team registered for camp. You can email us changes in count as the school year and summer progresses.
Please provide a summer email if you do not have access to your school email during the summer months. Important camp information and updated invoices will be sent to this email address till camp starts.

Emergency Contact Form - Please bring THREE copies of the form to registration. Everyone attending camp and staying on campus must be included on this form.

Camp Rules - Please read to each member of your squad and have them signed the signature page. Keep a copy for your self with the camp rules and turn the original in at registration.

**Release & Waiver Forms
Everyone attending camp must fill out a Release & Waiver forms and it must be completed and signed before camp.
I have highlighted information on both of the required forms. Click to view Adult or Participant form. The highlighted areas on these forms must be filled in and completed BEFORE camp registration. If you do not have insurance, you will need to write "SELF PAY" in the place where the insurance information goes.
Even the bus driver staying on campus will need to fill out the adult R&W form.
The director will need to make a copy of all the forms and keep the copies with them at all times during camp. The originals will be turned in at camp registration.
If your form has the $5.00 insurance, this is the wrong R&W form.
When: 10 AM-NOON. Opening Rally at 1:00 PM at the Mabee Athletics Center.
Where: Moody Center Lobby which is also where the cafeteria is located.
Resident Teams - Males
Male Team Members: Please be sure to include all male information on the form. All males will be housed separately. If you need special arrangements for the males, please let me know.
Male Coaches: You must bring a female sponsor to stay in the dorm with the team.

Helpful Information About Camp
Camp Schedule - Schedule is subject to change by the Head Instructor. If there are any changes, a new schedule will be handed out to you on the first day of camp during the first coaches meeting.

Dorm Room Assignment Form - You are welcome to use the attached form to set up your room assignments. You do not have to turn this in, it is for your benefit and not required. Deposits & Payments
Prices are for both individual and teams who would like to attend our camp.
In order to guarantee your bed and/or spot in camp, a $50.00 per person deposit is required. If the camp does become full and we have not received your deposit you may be asked to come as a commuter. If you need an invoice for just the deposit, please let me know and I will email you the invoice along with an invoice for the remaining balance.
Once we have received your registration information from NCA you will receive an email from us concerning camp. You will need to fill out our online registration form in order to finish registering for camp. Our online registration form contains information that we do not receive from NCA.
We will not automatically email invoices this year. Once you are ready for an invoice, please email or call us and I will be glad to email it to you. Please confirm your numbers in order to make sure your invoice is correct.
You will still need to inform us of any changes, example: change in count and contact information.
Once you have had tryouts, please email or call us with your new numbers. I will email you an updated invoice.
Refunds will not be available until after camp. Refunds are refunded back to the person or school that paid for camp.
Please send all payments to Southwest Camps NOT NCA.
You can pay all the registration fees in full beforehand, or you can pay just the deposit to hold your bed/spot at camp. Full payment is due at registration, this includes school purchase orders.
Payments mailed in must be in the form of an official school check, cashiers check, credit card or money order.
No personal, gym, booster club or business checks will be accepted after June 1, 2018.

If you have changes in participants or coaches that will be attending camp, please notify Southwest Camps immediately so we may update your account accordingly.

Resident Campers fee includes all meals during camp. Meals begin the first day (registration day) beginning at lunch and last ends the last day at breakfast.
Commuters are welcome to pay at the door of the cafeteria.
Dorm Room Assignments
Dorm name: Anderson Hall
Dorm rooms are assigned by HSU staff. You are welcome to use the above Dorm Room Assignment form to setup your rooming. This form is for your convenience and does not have to be turned in.
* Expect two to a room, four to a suite sharing a bathroom.
* Need Extra Long Twin size bed sheets for the bed.
* I recommend bringing a blanket just in case the room is too cold.
Special arrangements can be made for special needs or injuries through the camp coordinator, Carrie Marsh, Southwest Camps. Please call or email us so I can make arrangements.
If one of your dancers requires wrapping at all times, please be sure to bring your own wrapping to camp. The wrapping that we furnish at camp is for injuries during camp.

There will be a swim party on the evening of the third day of camp.
So bring your swimsuits! :)
Dorms - There can be NO decorating in the dorms.
Gym - You can decorate the walls in the gym. Duck tape is not allowed. Decorations must be removed at the end of camp.
Special Diet Arrangements for Campers
The cafeteria prepares meals/food for all different types of diet.
What to Wear to Camp
The statement below is a suggestion from NCA:
1st Day: Half day PM - Camp clothes - shorts and t-shirts/tank tops.
2nd Day: Full day - Camp clothes - shorts and t-shirts/tank tops.
3rd Day: Full day - Camp clothes - shorts and t-shirts/tank tops. This is also known as FUN DAY. You can dress to match the THEME but of course, they don't have to (just a suggestion!) This is also the day that participants try out for All American. Many kids choose to wear their uniforms for that.
4th Day: Half day AM - Most teams like to wear their uniforms on the final day but again, it is not required.

Individuals attending camp - If the participant is under the age of 18, he/she must bring a sponsor to camp to be with you at all times. The sponsor will need to attend the coaches/directors meeting each day too so that they will know what the participant will need to do for the day.
